After setting up an appointment date and time, I will ask you to fill out intake forms (available on my website) in preparation for our first session. During our first meeting, I will greet you at the office and we will have an initial consultation. The purpose of the initial session is for me to get a better understanding of your current concerns, any relevant background information, and treatment goals so that I can provide the best possible treatment. From there, if you feel that I will be a good fit for your needs, we will determine and schedule regular weekly sessions for on-going treatment.
My initial consultation fee is $200. My hourly fee is $175 and $150 for 45-minute individual sessions. I currently accept out-of-pocket payment in cash or check. Payment is due at the time of service.
I am currently an in-network provider for Horizon Blue Cross Blue Shield (PPO, HRA, HSA plans only). I am an out-of-network provider for all other insurances and am happy to provide the necessary information for insurance reimbursement. Please check with your insurance policy regarding mental health coverage and out-of-network benefits.
I have a 24 hour cancellation policy. If you need to cancel an appointment, please call me to let me know 24 hours prior to our scheduled session. If you do not provide 24 hour notice, you will be expected to pay for the missed session.
